Overfilled cheeks are why people ask "what have yo Overfilled cheeks are why people ask "what have you had done" instead of "you look great."

Two red flags to watch for in a consultation: being told that loading your cheeks with filler will fix your nasolabial folds by repositioning tissue, or that more cheek volume automatically creates a positive facial vector. Both sound logical. Neither gets you a natural result.

Nasolabial folds come from tissue descent, fat pad loss, and bony changes at the base of the nose. Trying to correct all of that through the cheek alone means overcorrecting one area to compensate for three. A little product placed across a few strategic areas will always outperform a lot of product in one.

This is what I would look like if I never touched This is what I would look like if I never touched medical aesthetics in my life.

Most people do not come in wanting to look different. They come in wanting to look like a rested, healthier version of themselves. That is exactly what I have been doing for years, addressing pigmentation with skincare and laser, improving skin texture and firmness through microneedling and radiofrequency, and reducing wrinkles while keeping things natural and masculine.

The goal is never to make someone look done. It is to make someone look great. If people are asking what you had done, something is off. The right result makes people say you look amazing and nothing else.

When all aspects of aging are addressed in a balanced way, that is when the magic happens. Join my Patreon for the full breakdown, link is in the bio.
We never chose to stop growing. We just chose to g We never chose to stop growing. We just chose to grow differently.

Most clinics scale by adding more providers, more rooms, more volume. We scaled by going deeper. Teaching plastic surgeons, dermatologists, physicians, nurses, and dentists internationally forced me to question everything about my own technique and decision-making. That knowledge comes right back to every patient who sits across from Michele and me.

No quotas. No handoffs. No pressure. Just two people who are genuinely obsessed with getting this right.

If you want to understand the thinking behind how we treat, teach, and make decisions, that conversation is happening on Patreon. Link is in the bio.
Needle placement in the lip is everything, and thi Needle placement in the lip is everything, and this is how you know you are in the right plane.

When you tent the needle outward, you are looking for one specific sign: white blanching of the skin. See gray and you are too superficial. No color change at all and you are too deep, sitting near the muscle. White blanch means you are exactly where you need to be, and you have to stay there for every single pass or the result will look uneven.

Entry point goes just inside the white roll, in the red, never on the border. Work your way down in one millimeter increments, thread down, thread up, and keep each quadrant to about 0.1 to 0.2 ml.

Precision like this is what separates a clean result from a correctable one.

I used to judge slow joggers. Turns out I was the I used to judge slow joggers. Turns out I was the problem.

As a lifelong gym rat I thought jogging slowly was pointless. Then I tried to jog consistently and could not make it past seven minutes. The only way I broke through that wall was by slowing down.

That person you see barely moving down the street is doing something most people never will. They showed up. They are pushing through the loudest mental resistance exercise has to offer, and they are not asking for your approval while doing it.

Consistency at any pace beats perfection on the couch every single time.

Rhinophyma is not caused by alcohol. It is caused Rhinophyma is not caused by alcohol. It is caused by chronic inflammation.

Most people assume the enlarged, thickened nose seen in rhinophyma comes from drinking. In reality, it is an advanced stage of rosacea. Alcohol can worsen it by dilating blood vessels and driving more inflammation, but the root cause is rosacea left unmanaged over time. Early intervention matters. Topicals like Rosatrol paired with zinc can help calm active rosacea before it progresses. For moderate to severe cases or established rhinophyma, laser treatments targeting superficial blood vessels combined with resurfacing can dramatically improve the skin. Prevention and consistency are everything with a chronic condition like this.

Putting diaper rash cream on your face sounds ridi Putting diaper rash cream on your face sounds ridiculous until you understand the science behind it.

Zinc oxide is the opposite of retinol. Where retinol works by irritating the skin to stimulate collagen, zinc oxide does the opposite. It calms redness, repairs your skin barrier, and acts as an occlusive to seal moisture in overnight. So when people wake up looking hydrated and glowy after trying this, they are not imagining it.

The way I recommend using it is simple. Use your retinoid five nights a week, and on your two rest nights, swap in a zinc oxide product instead. Your skin gets the recovery time it needs, and your results will actually improve.

Products to use:

Sudocreme
• Contains zinc oxide.
• Much easier to spread than Penaten.
• Probably my favorite "social media trend" alternative because it still contains zinc but feels less like you're frosting a cake.

Desitin
• Contains 13-40% zinc oxide depending on the version.
• More common in the US.
Contains Zinc (but not necessarily Zinc
Oxide)

Avène Cicalfate+
• Contains zinc sulfate and copper sulfate.
• Helps calm skin and support repair.
• Much more elegant cosmetically than diaper cream.
Lasers are not just a cosmetic treatment. They are Lasers are not just a cosmetic treatment. They are a health investment most people have never thought about.

Every time your skin is exposed to UV light, it creates tiny DNA errors called thymine dimers. Over time, as your skin's ability to repair itself slows down, those errors show up as pigmentation, wrinkles, texture changes, and in some cases, pre-cancerous lesions.

Laser technology does not just improve how your skin looks. It removes years of accumulated sun damage and replaces it with healthier cells, reducing your long-term risk of skin cancer in the process.

Looking better is the bonus. Healthier skin is the goal.

Radiesse might be the most underused filler in aes Radiesse might be the most underused filler in aesthetic medicine, and here is why that needs to change.

Pure Radiesse restructures. It sharpens the jaw, builds the chin, straightens the nose, and restores cheek projection without a single drop of HA. Dilute Radiesse goes a step further, stimulating collagen and elastin to firm the skin and slow tissue descent over time.

The catch? There is no dissolver. That is why I intentionally treat 50 to 60 percent of the desired outcome in session one, let it settle for two to three months, then refine from there. It is a safer, smarter approach that still delivers visible results far faster than most biostimulators.

If you are a provider who wants to learn how I integrate this into clinical practice, join me on Patreon. Link is in the bio.
Sculptra and dilute Radiesse are not the same thin Sculptra and dilute Radiesse are not the same thing, and the timeline tells you everything.

Both stimulate collagen, but the mechanisms are completely different. Dilute Radiesse uses calcium to trigger elastin and collagen production, while also giving you a temporary CMC gel volumizer that lasts three to four months. Sculptra uses poly-L-lactic acid particles with zero immediate volume, a short inflammatory phase, and then a gradual collagen response over two to four months. Both taper off around 24 to 36 months, but Sculptra edges out slightly longer in duration.

Knowing the why behind each product helps you have a smarter conversation with your injector and set realistic expectations for your results.

Three laser passes and your skin is basically a co Three laser passes and your skin is basically a completely different organ by the end.

The Erbium YAG is a full ablative laser, meaning it removes the entire top layer of skin. Think of it like a controlled reset. It does everything CO2 does but with a slightly safer profile, especially when it comes to pigmentation risk. The Neo we use also goes beyond skin tightening. It actively targets acne and acne scarring at the same time. Each session builds on the last, and by the third treatment patients genuinely cannot believe what they are seeing.

Treating tear troughs without a plan is how you ge Treating tear troughs without a plan is how you get lumps, puffiness, and unhappy patients.

When someone has both a midface depression and a tear trough concern, the order of operations matters. I always restore the cheek first. That foundation lifts the tissue, reduces how much work the tear trough actually needs, and gives me a cleaner canvas. Then I address the tear trough, and if there is still a subtle transition issue, I use whatever remains from that same thin filler to fan superficially and blend the junction.

The key is restraint. I stay within 0.5 ml total for that superficial zone on each side. Enough to refine, not enough to overload the lymphatics.

Sequence and volume discipline are everything in this area. If you want the full breakdown of how I approach complex midface cases, join my Patreon through the link in my bio.
Skipping the tail of the brow when treating the up Skipping the tail of the brow when treating the upper face is one of the most common mistakes I see.

The corrugator and orbicularis oculi are connected. They function as one unit. So when you only treat the central glabella and leave the lateral brow untouched, the tail compensates every time the patient squints and it pulls down.

By placing a small amount of neuromodulator at the peak and the tail, you recruit the full muscle group and get a genuine brow lift rather than a flat or heavy result.

Small adjustments, big difference in the outcome.

Can GLP-1 medications affect your ADHD symptoms? T Can GLP-1 medications affect your ADHD symptoms? There is a fascinating discussion happening right now about how these drugs might interact with the ADHD brain. While many people notice a helpful reduction in food noise, there is also a potential impact on dopamine seeking behaviors. Because ADHD is closely tied to dopamine and reward pathways, medications that dampen these systems might lead to less impulsivity, fewer cravings, and less scrolling. However, some people are also reporting a feeling of flatness or a loss of excitement for things they usually enjoy, almost as if their reward system has been turned down too much. It is important to remember that this is largely based on anecdotal reports and early theories, not hard peer-reviewed evidence. There is no definitive proof yet that GLP-1s treat ADHD or cause depression, but researchers are actively looking into it. Have you noticed any positive or negative changes in your focus, mood, or impulses while taking these medications? Let me know your thoughts and experiences in the comments.
The truth about before and after photos in the aes The truth about before and after photos in the aesthetics industry. It is surprisingly common for providers to manipulate their results. Some of the most frequent techniques include repositioning a patient's posture, such as tilting the head down in the before photo and higher in the after to artificially stretch the neck and sharpen the jawline.

Many also use different camera angles to create the illusion of a lift, apply makeup only in the after shots to hide bruising or enhance features, and use editing tools for subtle blemish smoothing to erase acne and uneven skin tone. The biggest and most widespread trick is manipulating the lighting, intentionally making the before photo darker and duller, while the after photo is brighter and far more flattering.

This deceptive practice is exactly why countries like Australia are cracking down on medical advertising, and it makes it incredibly hard for ethical injectors to compete. Once you know what to look for, you will start noticing it everywhere. I

Yes, it is a real thing. The medical aesthetics in Yes, it is a real thing. The medical aesthetics industry is buzzing about injectables made from purified cadaver fat matrix. They remove the fat cells, keep the structural proteins, and inject it to stimulate your body to grow its own fat.

It is a fascinating concept that sits somewhere between a bio-stimulator and a fat transfer, but there are a few major unanswered questions before I would ever consider using it.

First, you cannot dissolve it. If your body produces too much fat, it is permanent and would require surgical removal.

Second, we do not know how weight loss or GLP-1 medications will affect the new fat over time. We need to know if it will shrink and sag like metabolic fat.

Third, we do not know if the donor fat was metabolic or structural. This completely changes how it might behave when injected into your face.

Even if this was approved in Canada today, I would wait a good two to three years for real-world studies before offering it to my patients. Safety and predictable results always come first.

Stop falling for the viral Sculptra Facelift myth! Stop falling for the viral Sculptra Facelift myth!

Let’s get real for a second. Sculptra is an incredible treatment, but it does NOT physically pull your tissues back like a surgical facelift. Don't let exaggerated claims fool you.

Here’s the actual science behind what Sculptra does:

It’s a bio-stimulator (PLLA): It tells your body to build its own collagen and elastin.

Restores Volume: It naturally rebuilds lost facial volume over a series of sessions.

The Sculptra Glow: Increased circulation from collagen production gives your skin a real, radiant boost.

Structural Support: It reinforces the skin, SMAS, and muscle to prevent your tissues from slipping forward.

Sculptra is a marathon, not a sprint. The natural-looking results are entirely worth the wait.

Restoring the male cheek requires a completely dif Restoring the male cheek requires a completely different strategy than a female cheek. As I demonstrated at our recent EH-List Injector Series conference, the male apex is naturally lower and more medial.

To achieve a natural look, I first identify the intersection of the tear trough and zygomaticus ligaments. My goal is to restore volume and establish that low apex before working around the transition zones. If there is a depression behind the apex, I add product there to ensure the cheek tapers smoothly and looks structural rather than filled.

Precision in these transitions is what prevents an overdone or feminized appearance in male patients.

When I first started, I was taught to place my ent When I first started, I was taught to place my entry point for the marionette zone right in the jowl area. After a few weeks, some patients would return saying they could feel the filler had moved. They were right. The product was migrating back toward the entry point.

The issue is that a cannula creates a temporary channel in the tissue. Because the marionette area is so mobile. Think of all the talking, chewing, and mimicking we do. The movement was pushing the filler back through that channel and into the jowls.

To fix this, I changed my entrance point to be in front of the mandibular ligament. By placing the product and the channel entirely in front of the ligament, I’ve prevented the filler from being pushed back into the jowl area. The result is much more stable and stays exactly where it’s intended.

Lower face rejuvenation is one of my favourite tre Lower face rejuvenation is one of my favourite treatments because when done strategically and naturally, it can restore a tremendous amount of youthfulness without looking overfilled ✨

A common misconception is that treating only one area always looks more natural. In reality, if I had only filled the marionette zone here, I would have needed much more product in that area alone to achieve a similar improvement. Instead, volume was conservatively restored where it was actually lost.

This client had moderate to severe marionette hollowing with early jowling, and the goal was simple: restore structure naturally, not “blow up” the face 🙌🏻

Natural was always the plan.
