Do collagen peptides actually work for your skin? Do collagen peptides actually work for your skin? A recent meta-analysis of 24 randomized control trials, involving over 1,400 participants, suggests they do. The data showed measurable improvements in skin hydration, elasticity, and wrinkle reduction after 8 to 12 weeks of consistent use, particularly in aging or drier skin.

However, there are a few important caveats. When researchers isolated higher-quality studies and removed potential bias from industry-funded research, the results were less dramatic. Additionally, many of these supplements are combined with other active ingredients like zinc, biotin, and vitamin C, making it difficult to determine exactly how much of the improvement is due to the collagen alone.

The takeaway? Collagen supplements, specifically hydrolyzed peptides, likely offer benefits for hydration and elasticity, but the industry claims may be slightly overstated. We still need more long-term, independent data to know the full extent of their impact.

Sculptra versus Radiesse: where to inject and wher Sculptra versus Radiesse: where to inject and where to avoid.

Many providers lump these two biostimulators together, but they behave very differently in the tissue. Both work well in the temples, cheeks, jawline, and marionette zone because those areas provide enough support for predictable collagen stimulation.

I generally avoid both products around the eyes, mouth, nose, and forehead. With Sculptra, I am also much more cautious in the neck, decollete, and above the knees. These are tight or highly mobile areas where product clumping can lead to nodules.

Sculptra is poly-L-lactic acid, which works by triggering a controlled inflammatory response. Radiesse is calcium hydroxylapatite, which mimics your own bone and contains a gel that helps prevent clumping in mobile areas.

The most important thing to remember is that while the product matters, the provider matters more. Radiesse can settle in the syringe if left too long, and Sculptra requires precise placement in the correct tissue plane to avoid complications.

The skill of the injector is the biggest variable in your result.

Sculpting the jawline requires a tailored approach Sculpting the jawline requires a tailored approach, especially when working to balance out facial heaviness. For this patient, there is a slight heaviness in the front of the face due to buccal fat, so the goal is to highlight the gonial angle and add projection without dropping his natural jawline.

To achieve this, I use a combination technique. First, I place the needle directly on the bone at the gonial angle, injecting a small bolus of 0.2 to 0.3 to create that essential projection.

Next, I transition to a cannula, using a fanning technique in the subcutaneous region to match the jawline.

Why combine these methods? Relying on just one technique can sometimes leave a sharp demarcation or visible filler when the patient opens their jaw. By combining deep boluses on the bone with a superficial cannula fan, the product blends seamlessly into the tissue. This provides a highly accurate, natural-looking result without the risk of visible product.

I prefer a long 25 gauge, 2-inch cannula. It allows you to reach the midline much more easily and maintains better control throughout the treatment without having to create multiple entry points.

I am using my smart finger to feel exactly where the product is being placed and constantly oscillating the cannula to ensure even distribution and safety.

When it comes to filler, deep placement is key for When it comes to filler, deep placement is key for a natural, lifted result. Here is my approach to maximizing lift while keeping movement natural:

Go Deep: Whether using a cannula or a needle, I aim to place the majority of the product deep, right on the bone.

Less is More: By using a thicker product deep down, I can use less volume overall to achieve the necessary lift and tension.

The Sandwich Approach: After getting that deep, structural lift, I can fine-tune superficially if needed. I might take a tiny amount of the thicker product (blended with saline to soften it) or a softer product (like what I’d use for a tear trough) to lay gently on top.

Mind the Superficial Layer: I am always very careful with how much product goes into that superficial layer to ensure everything moves naturally!

Biostimulators & Autoimmune Disorders (Part 3 of 3 Biostimulators & Autoimmune Disorders (Part 3 of 3)

When treating patients with an autoimmune condition, a conservative approach is essential to keep the immune system balanced. Here is the exact safety protocol we use in our clinic:

Hyper-Dilution
Rather than a standard mix, we use a 1:2 dilution ratio with sterile saline. This disperses the calcium microspheres perfectly—providing the mechanical signal to build collagen without overwhelming the tissue or triggering an exaggerated immune response.

Conservative Dosing
We always start with a single syringe distributed globally across both sides of the face. This allows us to safely assess the patient's individual tolerance before proceeding with further treatment.

Clinical Monitoring
We require a 2 to 3 month wait before considering a second session. During this window, we closely monitor for any signs of an exaggerated local response, like prolonged swelling or nodules, as well as any systemic thyroid symptoms.

In aesthetic medicine, patient physiology must always dictate the treatment plan.

Both are incredible biostimulators, but their mechanisms of action are very different. If you have an autoimmune condition like Hashimoto’s, understanding this difference is crucial. Here is the clinical breakdown:

Sculptra (Poly-L-Lactic Acid)
Sculptra stimulates collagen production by creating a mild, controlled inflammatory response. While this is highly effective for the average patient, intentionally introducing inflammation can carry higher risks for someone whose immune system is already overactive.

Radiesse (Calcium Hydroxylapatite)
Radiesse is composed of calcium-based microspheres, meaning the body recognizes it much like native bone matrix. It acts as a localized scaffold, allowing your cells to build collagen naturally without triggering a widespread systemic immune response.

The Clinical Takeaway
When treating autoimmune patients, maintaining immune balance is our top priority. Radiesse allows us to achieve structural support and natural collagen stimulation while keeping the risk of a flare-up to an absolute minimum.

A frequent clinical inquiry we receive during consultation is whether patients with autoimmune conditions, specifically Hashimoto’s thyroiditis, are suitable candidates for collagen-stimulating treatments.

Biostimulators traditionally rely on triggering a controlled inflammatory cascade to stimulate fibroblasts and produce new collagen. However, when a patient's immune system is already dysregulated—such as actively targeting the thyroid gland—we must approach treatment with extreme care. Introducing a generalized immune response in these patients carries a higher risk of triggering undesirable outcomes, such as joint pain, excessive localized swelling, or systemic flare-ups.

An autoimmune diagnosis is not an absolute contraindication for biostimulation, but it does mandate a highly modified clinical approach.

If we over-tox the lower crow's feet area, we block the muscle from lifting the cheek when they smile. The goal is always a genuine Duchenne smile (smiling with the eyes!). We achieve this by using low doses (just 1 or 2 units) strategically placed higher up, softening the lines without freezing the natural cheek lift. 📈

Myth: Deep Sculptra injections improve surface-level skin elasticity. Fact: Collagen stimulation only happens at the level the product is injected.

Myth: All fillers are "toxic" and you should only do fat transfers. Fact: Many providers bash injectables simply because they don't offer them in their practice. It’s branding, not medicine.

Never be afraid to ask your provider questions! My goal is to always give you the transparent pros and cons.

While dermal fillers don’t physically pull tissue back like a surgical facelift, they can create a dramatic lifting illusion when injected with the X, Y, and Z axes of facial aging in mind.

Instead of just chasing the "Z-axis" (deflation) and overfilling the face outward, we have to use structural products (like Radiesse or Sculptra) on receded bony prominences to: 1️⃣ Pull the forward-moving tissue taut. 2️⃣ Support descending tissue from below. 3️⃣ Recreate the bone structure lost to aging.
When injectors fail to assess all three axes, you get unnatural, distorted results.'

As I break down in the video, it all comes down to the 500 Dalton Rule. 

Retinol goes deep into the dermis to stimulate collagen and break up pigment.

Peptides are larger molecules that stay in the epidermis (your skin’s armor). They’re amazing for hydration and barrier protection, but they don’t reach the deep layers where aging actually happens.

The "Replacement" Routine: If you can’t tolerate retinol, here is the 4-step formula I recommend for my clients:

Ever seen these tiny little lines? 💉🧐 We call them Ever seen these tiny little lines? 💉🧐 We call them “Commas,” and they pop up right above the eyebrow arch after a neuromodulator treatment.

The secret is depth and dose! 📉 We intentionally leave that area untreated to protect the brow arch and allow for movement. But if we want to erase them without causing the brow to drop, the trick is to use a very minute micro-dose (just 0.5 to 1 unit!) placed superficially (intradermally) right above the arch. This targets the wrinkle without hitting the deeper muscle that causes the brow lift.
