Sculptra and dilute Radiesse are not the same thin Sculptra and dilute Radiesse are not the same thing, and the timeline tells you everything.

Both stimulate collagen, but the mechanisms are completely different. Dilute Radiesse uses calcium to trigger elastin and collagen production, while also giving you a temporary CMC gel volumizer that lasts three to four months. Sculptra uses poly-L-lactic acid particles with zero immediate volume, a short inflammatory phase, and then a gradual collagen response over two to four months. Both taper off around 24 to 36 months, but Sculptra edges out slightly longer in duration.

Knowing the why behind each product helps you have a smarter conversation with your injector and set realistic expectations for your results.

The Erbium YAG is a full ablative laser, meaning it removes the entire top layer of skin. Think of it like a controlled reset. It does everything CO2 does but with a slightly safer profile, especially when it comes to pigmentation risk. The Neo we use also goes beyond skin tightening. It actively targets acne and acne scarring at the same time. Each session builds on the last, and by the third treatment patients genuinely cannot believe what they are seeing.

When someone has both a midface depression and a tear trough concern, the order of operations matters. I always restore the cheek first. That foundation lifts the tissue, reduces how much work the tear trough actually needs, and gives me a cleaner canvas. Then I address the tear trough, and if there is still a subtle transition issue, I use whatever remains from that same thin filler to fan superficially and blend the junction.

The key is restraint. I stay within 0.5 ml total for that superficial zone on each side. Enough to refine, not enough to overload the lymphatics.

Skipping the tail of the brow when treating the up Skipping the tail of the brow when treating the upper face is one of the most common mistakes I see.

The corrugator and orbicularis oculi are connected. They function as one unit. So when you only treat the central glabella and leave the lateral brow untouched, the tail compensates every time the patient squints and it pulls down.

By placing a small amount of neuromodulator at the peak and the tail, you recruit the full muscle group and get a genuine brow lift rather than a flat or heavy result.

Small adjustments, big difference in the outcome.

While many people notice a helpful reduction in food noise, there is also a potential impact on dopamine seeking behaviors. Because ADHD is closely tied to dopamine and reward pathways, medications that dampen these systems might lead to less impulsivity, fewer cravings, and less scrolling. However, some people are also reporting a feeling of flatness or a loss of excitement for things they usually enjoy, almost as if their reward system has been turned down too much. It is important to remember that this is largely based on anecdotal reports and early theories, not hard peer-reviewed evidence. There is no definitive proof yet that GLP-1s treat ADHD or cause depression, but researchers are actively looking into it. The truth about before and after photos in the aes The truth about before and after photos in the aesthetics industry. It is surprisingly common for providers to manipulate their results. Some of the most frequent techniques include repositioning a patient's posture, such as tilting the head down in the before photo and higher in the after to artificially stretch the neck and sharpen the jawline. 

Many also use different camera angles to create the illusion of a lift, apply makeup only in the after shots to hide bruising or enhance features, and use editing tools for subtle blemish smoothing to erase acne and uneven skin tone. The biggest and most widespread trick is manipulating the lighting, intentionally making the before photo darker and duller, while the after photo is brighter and far more flattering. 

This deceptive practice is exactly why countries like Australia are cracking down on medical advertising, and it makes it incredibly hard for ethical injectors to compete. Once you know what to look for, you will start noticing it everywhere. I

Yes, it is a real thing. The medical aesthetics in Yes, it is a real thing. The medical aesthetics industry is buzzing about injectables made from purified cadaver fat matrix. They remove the fat cells, keep the structural proteins, and inject it to stimulate your body to grow its own fat.

It is a fascinating concept that sits somewhere between a bio-stimulator and a fat transfer, but there are a few major unanswered questions before I would ever consider using it.

First, you cannot dissolve it. If your body produces too much fat, it is permanent and would require surgical removal.

Second, we do not know how weight loss or GLP-1 medications will affect the new fat over time. We need to know if it will shrink and sag like metabolic fat.

Third, we do not know if the donor fat was metabolic or structural. This completely changes how it might behave when injected into your face.

Even if this was approved in Canada today, I would wait a good two to three years for real-world studies before offering it to my patients. Safety and predictable results always come first.

Restoring the male cheek requires a completely dif Restoring the male cheek requires a completely different strategy than a female cheek. As I demonstrated at our recent EH-List Injector Series conference, the male apex is naturally lower and more medial.

To achieve a natural look, I first identify the intersection of the tear trough and zygomaticus ligaments. My goal is to restore volume and establish that low apex before working around the transition zones. If there is a depression behind the apex, I add product there to ensure the cheek tapers smoothly and looks structural rather than filled.

Precision in these transitions is what prevents an overdone or feminized appearance in male patients.

When I first started, I was taught to place my ent When I first started, I was taught to place my entry point for the marionette zone right in the jowl area. After a few weeks, some patients would return saying they could feel the filler had moved. They were right. The product was migrating back toward the entry point.

The issue is that a cannula creates a temporary channel in the tissue. Because the marionette area is so mobile. Think of all the talking, chewing, and mimicking we do. The movement was pushing the filler back through that channel and into the jowls.

To fix this, I changed my entrance point to be in front of the mandibular ligament. By placing the product and the channel entirely in front of the ligament, I’ve prevented the filler from being pushed back into the jowl area. The result is much more stable and stays exactly where it’s intended.

A common misconception is that treating only one area always looks more natural. In reality, if I had only filled the marionette zone here, I would have needed much more product in that area alone to achieve a similar improvement. Instead, volume was conservatively restored where it was actually lost. 

This client had moderate to severe marionette hollowing with early jowling, and the goal was simple: restore structure naturally, not “blow up” the face 🙌🏻

Natural was always the plan.

Do collagen peptides actually work for your skin? Do collagen peptides actually work for your skin? A recent meta-analysis of 24 randomized control trials, involving over 1,400 participants, suggests they do. The data showed measurable improvements in skin hydration, elasticity, and wrinkle reduction after 8 to 12 weeks of consistent use, particularly in aging or drier skin.

However, there are a few important caveats. When researchers isolated higher-quality studies and removed potential bias from industry-funded research, the results were less dramatic. Additionally, many of these supplements are combined with other active ingredients like zinc, biotin, and vitamin C, making it difficult to determine exactly how much of the improvement is due to the collagen alone.

The takeaway? Collagen supplements, specifically hydrolyzed peptides, likely offer benefits for hydration and elasticity, but the industry claims may be slightly overstated. We still need more long-term, independent data to know the full extent of their impact.

Sculptra versus Radiesse: where to inject and wher Sculptra versus Radiesse: where to inject and where to avoid.

Many providers lump these two biostimulators together, but they behave very differently in the tissue. Both work well in the temples, cheeks, jawline, and marionette zone because those areas provide enough support for predictable collagen stimulation.

I generally avoid both products around the eyes, mouth, nose, and forehead. With Sculptra, I am also much more cautious in the neck, decollete, and above the knees. These are tight or highly mobile areas where product clumping can lead to nodules.

Sculptra is poly-L-lactic acid, which works by triggering a controlled inflammatory response. Radiesse is calcium hydroxylapatite, which mimics your own bone and contains a gel that helps prevent clumping in mobile areas.

The most important thing to remember is that while the product matters, the provider matters more. Radiesse can settle in the syringe if left too long, and Sculptra requires precise placement in the correct tissue plane to avoid complications.

The skill of the injector is the biggest variable in your result.

Sculpting the jawline requires a tailored approach Sculpting the jawline requires a tailored approach, especially when working to balance out facial heaviness. For this patient, there is a slight heaviness in the front of the face due to buccal fat, so the goal is to highlight the gonial angle and add projection without dropping his natural jawline.

To achieve this, I use a combination technique. First, I place the needle directly on the bone at the gonial angle, injecting a small bolus of 0.2 to 0.3 to create that essential projection.

Next, I transition to a cannula, using a fanning technique in the subcutaneous region to match the jawline.

Why combine these methods? Relying on just one technique can sometimes leave a sharp demarcation or visible filler when the patient opens their jaw. By combining deep boluses on the bone with a superficial cannula fan, the product blends seamlessly into the tissue. This provides a highly accurate, natural-looking result without the risk of visible product.

I prefer a long 25 gauge, 2-inch cannula. It allows you to reach the midline much more easily and maintains better control throughout the treatment without having to create multiple entry points.

I am using my smart finger to feel exactly where the product is being placed and constantly oscillating the cannula to ensure even distribution and safety.
